I've had flawless or near flawless runs, same thing. They are bugged in several ways. Can confirm #1 is likely the bugged one as I've been able to get the chest just fine if #2 is one of the 3 (but not #1). I personally shot all the symbols in 2 runs I had yesterday (1, 2, 3 and then 1, 2, 6), no wipes anywhere, and it still didn't activate. You have to double check every symbol. And after every wipe with a symbol in the encounter room the symbol resets.
